Sports Sign-Up Link

In order to participate in sports there are a few forms that need to completed prior to being eligible to participate in school sports.  These following forms are all digital forms and will be sent to each grade level email groups and include the following:

  1. Permission to Participate Waiver
  2. Student Health Questionnaire Form
  3. Parent/Athlete Concussion Information Form

Additionally, your student/athlete will need to have a current, up to date sports physical on file with the school nurse in order to participate in practices or tryouts... so please, be sure there is a current sports physical on file with the school nurse, Stasia Cardillo, prior to Aug. 28, 2023. Once a student/athlete is signed I will cross-reference with forms from last year as well.

The sports webpage has a number of links to area Urgent Care or Sports medicine facilities that offer walk-in sports physical for a small fee if you are unable to get into your primary medical care provider prior to the start of school.

Click on any of the the facilities listed below to be directed to their website:

Enjoy the rest of your winter sports and see you on the ball fields or the track this spring!
